- Hepatitis B if not immune and indicated
- Live vaccines: discuss before treatment; many should be avoided during treatment.

CALL THE CLINIC URGENTLY FOR
- Fever or signs of serious infection
- Chest pain, shortness of breath, calf swelling/pain
- New shingles rash
- New bloody diarrhea or severe abdominal pain
- New severe mood change or suicidal thoughts
- New severe spine pain after a fall/collision, especially with a fused spine
